Discuss your concerns with your doctor and request a referral be sent to Discovering Expression Speech and Language  

2. Check insurance coverage 

Call your insurance to discuss your deductible, co-pays and total out of pocket coverage for the year. 

3. Schedule an evaluation

Connect with our Patient Care Coordinator by sending email to referrals@discoveringexpression.com or calling 360-890-8754 to schedule an evaluation.

4. Fill out intake forms

Once you’ve scheduled an evaluation appointment, fill out your child’s intake forms:

• Credit Card Agreement Form

• Developmental History Form

• HIPAA Consent Form

• Patient Intake Form

• Release for Electronic Communication

• Speech Therapy Disclosure Statement

• Consent to Release Confidential Medical Information

• Feeding Screener

5. Begin Services 

Work with our referral coordinator at (360) 890-8754 to find appointment times that work best for your schedule.
